- If available, please bring the following items to the initial appointment:
- Medical records: previous medical documents that are relevant to your diagnosis, including x-rays, test results, and consultation notes.
- Immunization records: a current list of vaccinations received.
- Please provide a detailed list of any allergies, including materials, foods, and medications.
- If necessary, obtain referral letters from general practitioners (GPs) or other physicians.
- If you have an appointment, it should be one hour after you register.
- On your second visit, bring your medical record card with you so that you can get your information. At the registration counter, provide your name and cell phone number in case you misplace your medical record card. After the initial evaluation, your medical team will create a personalized management plan. You will receive a detailed explanation of this strategy.
